MEDIA RELEASE

I hereby grant VT3 ENTERPRISES, LLC (hereinafter “VT3” including its assigns, subsidiaries, executors, licensees, and heirs) permission and absolute right to use my image or likeness in any and all of its publications, including, but not limited to, all of VT3’s printed and digital publications including portraits, pictures, videotapes, streaming content, social media, or websites without payment or any other consideration. I understand and agree that any photograph using my image or likeness will become property of VT3 and will not be returned.

I hereby release and hold harmless VT3 from any reasonable expectation of privacy or confidentiality associated with the image(s) specified above.

PLEASE READ THESE TERMS OF USE CAREFULLY BEFORE USING THE SITE. These Terms of Use constitute an agreement between REIMAGE Beauty and you. We recommend that you print out a copy of these Terms of Use for your records.I hereby irrevocably authorize VT3 to edit, alter, copy, exhibit, publish, or distribute this photo for purposes of publicizing VT3’s programs, promotions, campaigns, products, or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or likeness.

I hereby hold harmless and release and forever discharge VT3 from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or my estate may have by reason of this authorization.

This Agreement shall be governed and interpreted under the laws of the State of Illinois without regard to the conflicts of law provisions thereof.

In the event that any provision of this Agreement shall be determined by a court of competent jurisdiction to be illegal or unenforceable, that provision will be limited or eliminated to the minimum extent necessary so that this Agreement shall otherwise remain in full force and effect and enforceable.
